Records Clerk PIDG/RIM 1 (vacant)

Salary $1,272,269-1,711,060 per annum

JOB PURPOSE

Under the General supervision of the Administrator, the Records Clerk is required to locate and retrieve files and ensure that correspondence are accurately filed and files are maintained to facilitate easy access of information as well as the scheduling of patients for surgery and post-operative evaluations for the ophthalmology centre.

KEY RESPONSIBILITY AREAS
  • Ensure that files requested are dispatched and files are re-shelved
  • Sorts records
  • Maintains records of the movement of files
  • Ensure that all files and enclosures are numbered accordingly are in correct order
  • Files correspondence and charge out cards
  • Locate cards and replace on returned files
  • Creates new files
  • Maintains “Bring up” System
  • Removes inactive records for shelves
  • Prepares inactive records for transfer
  • Records and re-shelf incoming files
  • Overseas the operations of the ophthalmology centre records
  • Schedule patients for pre-operative, post-operative evaluations, surgery and Laser treatment.

MINIMUM REQUIRED EDUCATION AND EXPERIENCE
  • Four (4) CXC or O’ Level including English
  • Training in records Management

Health Records Clerk (St. Mary & Portland, Jamaica) - NERHA


 NORTH-EAST REGIONAL HEALTH AUTHORITY

HEALTH RECORDS CLERK (HTAC/HRT 1)

The North East Regional Health Authority, a statutory body under the Ministry of Health, with
responsibility for the management and delivery of public health services within the parishes of St. Ann, St. Mary, and Portland, is seeking to fill the position of Health Records Clerk at the St. Mary and Portland Health Departments.

The incumbent will be responsible for to compile, process and maintain the health records in a manner
consistent with medical administrative, ethical, legal and regulatory requirements of the health care facility.
Qualification and Experience will include:
  • Four (4) CXC/GCE subjects at the ordinary level (O’level) are required including English Language,
  • Mathematics and a Science subject (preferably Human and Social Biology).
  • Diploma in Computer applications inclusive of word processing and spreadsheet applications
  • A minimum of two (2) years’ experience in related field.

Specific Knowledge Required:
  • Sound knowledge of policies and practices involved in health records science
  • Health Records Service Manual
  • Medical Records Manual –: Guide for Developing Countries
  • Jamaica Medical Records Code of Ethics
  • Monthly Clinic Statistical Report (MCSR) Manual
  • Staff Order
Responsibilities will include but not limited to:
  • Protect the security of the health records, ensuring that confidentiality is maintained
  • Gathering of demographic, geographic and health related data and history from all new patients upon registration, asking all the prompt questions on the form
  • Preparing patient service record for the appropriate clinic
  • Entering of patient’s particular in Daily Register log book to facilitate tally of all registered patients
  • Entering of data on the sentinel forms for each registration
  • Entering of patient record particulars for retrieved records in the record request log book
  • Register all patients for the various clinics
  • Ensure that all health records are assigned to clinic locations
  • Retrieve all health records for appointment
  • Communicate with patient/client as the need arise in relation to their appointment
  • Ensure good health record keeping is maintained
  • Ensure proper maintenance of all health records
REMUNERATION PACKAGE PER ANNUM: Salary Scale: $ 1,657,751 – 1,970,545 per annum

Records Clerk (Clarendon, Jamaica) - Percy Junor Hospital (SRHA)

RECORDS CLERK 1 (PIDG/RIM 1- Pay Band 3) –Operations Division – Vacant (salary range $1,272,269 – $1,711,060 per annum and any allowance(s) attached to the post).

Job Summary

Under the supervision of the Operations Manager, the incumbent will be responsible for
providing clerical support to the Operations Manager as it relates to compiling of
reports, assisting in maintaining files and updating the Operation Software, assisting in
vetting claims, and providing other administrative support.

Minimum Qualification and Experience:
  • Minimum four (4) subjects at GCE O’Level/CXC General Proficiency level inclusive of English Language and Mathematics.

  • Two (2) years’ experience at a clerical level in Administration within a public sector organization.
Required Knowledge/Skills/Specialized Techniques:
  • Working knowledge of the Financial Administration and Audit Act (FAA Act)
  • Working knowledge of the Staff Orders
  • Working knowledge of accounting principles & practices
  • Knowledge of computer applications (Microsoft Office)

Key responsibilities will include:
  • Assists the Operations Manager in vetting claims
  • Maintains general records for Operations Manager by ensuring that information is kept accurate and up-to-date at all times.
  • Provides routine office support as making copies, faxing, answering telephones, prepares correspondence for mailing, and delivering and picking up correspondence.
  • Compiles reports from existing records including Drivers’ Checklist Daily Assignment
  • Takes and transcribes minutes of meetings: security, other contractors, project team etc.
  • Ensure queries of customers/visitors to the unit are received and referred/attended to in a timely manner using effective communication techniques.

SOUTH EAST REGIONAL HEALTH AUTHORITY

 We invite applications from suitably qualified persons for the position of Health Records Clerk (HTAC/HRT 1) - St. Thomas Health Department & St. Jago Park Health Centre. 


SCOPE OF WORK / BACKGROUND
Jamaica has an estimated prevalence of 1.6% among the general population. Based on modeled estimates and case based surveillance data, it is estimated that 29,000 persons are currently living with HIV in Jamaica. Approximately 84% of the estimated number is aware of their status. This is followed by a large gap in the continuum of care with 43% of those diagnosed on ARVs and 55% of those on treatment being virally suppressed.

The Health Records Officer is to ensure that the basic standards for health records management and patient health information systems are maintained and that optimum patient care within the framework of national priorities and objectives are met.

OBJECTIVE/SUMMARY FOR POSITION

Under the direct supervision of the facility’s Health Records Administrator/Technician, the Health Records Officer has the responsibility to compile process and maintain the health records in a manner consistent with medical, administrative, ethical, legal and regulatory requirements of the health care facility.

MAJOR DUTIES AND RESPONSIBILITIES

  • Protect the security of the health records, ensuring that confidentiality is maintained
  • Gathering of demographic, geographic and health related data and history from all new patients upon registration and three monthly basis, asking all the prompt questions on the form
  • Preparing patient service record for the appropriate clinic
  • Entering of patient’s particular in Daily Register log book to facilitate tally of all registered patients
  • Entering of data on the sentinel forms for each registration
  • Entering of patient record particulars for retrieved records in the record request log book
  • Register all patients for the various clinics
  • Ensure that all health records are assigned to clinic locations
  • Retrieve all health records for appointment
  • Communicate with the patient/client as the need arises in relation to their appointment
  • Ensure good health record keeping is maintained
  • Ensure proper maintenance of all health records by checking all records for
    • Proper identification on all forms
    • Correct sequence of all form by date and time
    • Make a health record for each new patient and retrieve old records for patients who were previously seen at facility
  • Reassemble discharged health records in chronological order
  • Attach loose reports to the record
  • Record all diagnosis or reason for visit in Daily Curative register
  • Make appointments for patients who are for follow-up
  • Answer the telephone
  • Sort and arrange all patient index and records for filing
  • File all health records according to numbering system and indexes in alphabetic order
  • Perform any other health records related duties assign from time to time.

QUALIFICATIONS AND EXPERIENCE
- Good typing skills (at least 30 words per minute)
- Working knowledge in the use of Windows based computers
- Working knowledge in Microsoft Office
Plus

- Five (5) subjects in CXC/ GCE including Mathematics and English ( Information
technology / computer studies would be a plus)

SPECIFIC KNOWLEDGE REQUIRED

  • Alpha – numeric filing
