We invite applications from suitably qualified persons for the position of Health Records Clerk (HTAC/HRT 1) - St. Thomas Health Department & St. Jago Park Health Centre.
SCOPE OF WORK / BACKGROUND
Jamaica has an estimated prevalence of 1.6% among the general population. Based on modeled estimates and case based surveillance data, it is estimated that 29,000 persons are currently living with HIV in Jamaica. Approximately 84% of the estimated number is aware of their status. This is followed by a large gap in the continuum of care with 43% of those diagnosed on ARVs and 55% of those on treatment being virally suppressed.
The Health Records Officer is to ensure that the basic standards for health records management and patient health information systems are maintained and that optimum patient care within the framework of national priorities and objectives are met.
OBJECTIVE/SUMMARY FOR POSITION
Under the direct supervision of the facility’s Health Records Administrator/Technician, the Health Records Officer has the responsibility to compile process and maintain the health records in a manner consistent with medical, administrative, ethical, legal and regulatory requirements of the health care facility.
MAJOR DUTIES AND RESPONSIBILITIES
- Protect the security of the health records, ensuring that confidentiality is maintained
- Gathering of demographic, geographic and health related data and history from all new patients upon registration and three monthly basis, asking all the prompt questions on the form
- Preparing patient service record for the appropriate clinic
- Entering of patient’s particular in Daily Register log book to facilitate tally of all registered patients
- Entering of data on the sentinel forms for each registration
- Entering of patient record particulars for retrieved records in the record request log book
- Register all patients for the various clinics
- Ensure that all health records are assigned to clinic locations
- Retrieve all health records for appointment
- Communicate with the patient/client as the need arises in relation to their appointment
- Ensure good health record keeping is maintained
- Ensure proper maintenance of all health records by checking all records for
- Proper identification on all forms
- Correct sequence of all form by date and time
- Make a health record for each new patient and retrieve old records for patients who were previously seen at facility
- Reassemble discharged health records in chronological order
- Attach loose reports to the record
- Record all diagnosis or reason for visit in Daily Curative register
- Make appointments for patients who are for follow-up
- Answer the telephone
- Sort and arrange all patient index and records for filing
- File all health records according to numbering system and indexes in alphabetic order
- Perform any other health records related duties assign from time to time.
QUALIFICATIONS AND EXPERIENCE
- Good typing skills (at least 30 words per minute)
- Working knowledge in the use of Windows based computers
- Working knowledge in Microsoft Office
Plus
- Five (5) subjects in CXC/ GCE including Mathematics and English ( Information
technology / computer studies would be a plus)
