Job Summary:
Under the supervision of the Health Records Technician and the technical supervision of the
Parish Health Records Administrator, the incumbent will have the responsibility of
organizing, implementing, controlling and maintaining the patient information system for the
provision of optimum patient care.

Qualifications and Experience:

  • Four (4) ‘O’ Level/CXC subjects; including passes in English Language, Mathematics & Biology
  • Clerical and/or computer experience is an asset

Specific Knowledge and Required Skills:
  • Good knowledge of the methods, procedures, practices, rules and regulations related of health data collection
  • Good knowledge of Computer Applications
  • Ability to analyse statistical reports and make recommendations
  • Good interpersonal communication skills
  • Good conflict resolution skills

Key responsibilities include:
  • Protecting the security of the health records to ensure confidentiality is maintained.
  • Developing and maintaining efficient and accurate filing and retrieval of health records.

  • Filing laboratory/ other investigation reports and referral forms in patient/client health record.

  • Compiling and submitting Monthly Clinic Summary Reports, Sentinel, STD Syndrome and other reports.
  • Maintaining the appointment system for the various clinics held. Maintaining the Primary
  • Health Care Register for the health records department manually and electronically
  • Educating other members of the health team on the importance of health records, with clear definition of health records duties.
  • Assisting with the smooth running of the clinics; directing patients, developing a system whereby the records are transferred from the filing area to the clinic area.
  • Responsible for the acquisition and monitoring of health records stationery/equipment.
  • Completing the list for the PATH programme.
  • Collecting and compiling of data for clinical activities done are to be recorded daily on the specific tally sheets.
  • Retrieving health records to facilitate researches by authorized users/personnel.

Health Records Technician (St. Ann, St. Mary & Portland - Jamaica) - NERHA

The North-East Regional Health Authority, a statutory body under the Ministry of Health and Wellness, with responsibility for the management and operation of public health services within the parishes of St. Ann, St. Mary, and Portland is seeking a dynamic, proactive and experienced individual to fill the position of Health Records Technician 1 (HTAC/HRT 2) for Hospitals and Health Departments in St. Ann, St. Mary and Portland.

Reporting to the Health Records Administrator, the ideal candidate must meet the following minimum:
Qualifications:
  • Associate Science Degree in Health Information Technology or Certificate in Health Records and Statistics Level 11
  • At least three (3) years full-time progressive experience in work related to the preparation and maintenance of health records and health statistics. One of which should be at supervisory level.
Required Skills and Specialized Techniques:
  • Ability to analyze statistical reports and make recommendations
  • Skills in Interpersonal Communication
  • Skills in Conflict Resolution

Job Summary:
The Health Records Clerk under the supervision of the Health Records Administrator, has the responsibility for organizing, implementing, controlling and maintaining the patient information system for the provision of optimum patient care.

Minimum Required Education and Experience:

  • Assembling records after registration of patients before referring patients to the Cashier.
  • Educating patients on the importance of keeping their appointment date and taking the appointment card when returning to the clinic.
  • Preparing for the clinic by ensuring that all health records for patients to attend on a particular date be retrieved before the date of the clinic.
  • Ensuring that out guides are placed in the file for each record retrieved.
  • Filing laboratory and x-ray results in the appropriate health records and adds continuation sheets to records if needed the day before clinic.
  • Ensuring that all the health records needed for the clinic are found before the day of the clinic.
  • Completing an inventory for all the records retrieved for the clinic and places a check mark beside the records that are found.
  • Preparing numbers to be issued to patients on arrival at the clinic so as to maintain order in the waiting area.

  • Maintaining of all methods, systems and policies which enhances efficient running of the Department

JOB PURPOSE

Under the General supervision of the Administrator, the Records Clerk is required to locate and retrieve files and ensure that correspondence are accurately filed and files are maintained to facilitate easy access of information as well as the scheduling of patients for surgery and post-operative evaluations for the ophthalmology centre.

KEY RESPONSIBILITY AREAS
  • Ensure that files requested are dispatched and files are re-shelved
  • Sorts records
  • Maintains records of the movement of files
  • Ensure that all files and enclosures are numbered accordingly are in correct order
  • Files correspondence and charge out cards
  • Locate cards and replace on returned files
  • Creates new files
  • Maintains “Bring up” System
  • Removes inactive records for shelves
  • Prepares inactive records for transfer
  • Records and re-shelf incoming files
  • Overseas the operations of the ophthalmology centre records
  • Schedule patients for pre-operative, post-operative evaluations, surgery and Laser treatment.

HEALTH RECORDS CLERK (HTAC/HRT 1)

The North East Regional Health Authority, a statutory body under the Ministry of Health, with

The incumbent will be responsible for to compile, process and maintain the health records in a manner
consistent with medical administrative, ethical, legal and regulatory requirements of the health care facility.
Qualification and Experience will include:

HEALTH RECORDS TECHNICIAN (HTAC/HRT 2)
Two (2) Vacant Positions
(Salary range $ 2,585,526 - $ 3,073,378 per annum plus any allowance (s) attached to the post).

Job Purpose

Under the direct supervision of the Health Records Administrator, the incumbent has the responsibility to perform health records services in secondary health care facilities in one of the main areas of the Health Records Department: Medico-Legal, File Room, Statistics, Medical Data, Classification (Coding), Wards, Death Registry, Registration, Accident & Emergency, Outpatients, Admission, Discharge.

Qualification and Experience
  • Associate Science Degree in Health Information Technology or Certificates in Health Records and Statistics Level 11 and Level I
  • Three (3) years full-time progressive experience in work related to the preparation and maintenance of health records and health statistics. One of which should be at supervisory level.
OR
  • Any equivalent combination of education and experience.
  • Certificate in Supervisory Management

Key Responsibilities will include:

Technical/Professional
  • Collecting Daily inpatient Census and ensure information is correct
  • Ensuring all admission and discharge patients name are entered on census
  • Checking names of patients transferred or admitted but not entered in Register.
  • Checking the records of all discharge patients to ensure all are entered on the census
  • Transcribing information form Daily Inpatient Census on daily summary (Tally) Sheet
  • Summarizing all information and enter same on HMSR forms.
  • Collecting all discharge patient files after the Quantitative and Qualitative analysis is completed.
  • Preparing discharge activity control list
  • Using ICD coding manuals to assign an internationally accepted code to both diagnosis and procedures
  • Writing codes in the provided section of patient’s record
  • Completing Medical Case Abstract Form indicating diagnosis, procedures, assigned codes, record number and admission and discharge dates.
  • Collecting completed application forms from Police Officer
  • Locating relevant patient’s record
  • Issuing form to the Police Officer and ensure signature is received and ID presented.
  • Collecting letters of request from patients, Attorneys, Insurance Companies, Court, Immigration Bodies or High Commissions and other authorize agents.
  • Ensuring that patient’s signs consent for release of information to requesting body.
  • Completing death register.
  • Notifying Pathologist of post mortems to be conducted.
  • Ensuring completion of authorized forms for release and disposal of bodies.
  • Ensuring the proper and timely completion of the Medical Certificate of Cause of Death Form from the Medical Officer.
  • Developing and maintaining an efficient and accurate standardized filing system for filing and retrieval of medical records.
  • Protecting the security of the health records to ensure confidentiality is maintained
  • Ensuring files are protected.
  • Completing inpatient census and head count of each patient admitted on the wards.
  • Assisting the clinicians by ensuring the proper maintenance of health records for inpatients.
Special Conditions Associated with the Job:
  • May require the movement of items weighing up to 15kg with and without the use of mechanical aids;
  • High levels of concentration required to ensure reliable results/service and safe product to clients;
  • Daily exposure in a controlled environment to contained and uncontained blood samples/donations, potentially infected material and contaminated equipment;
  • Daily exposure to low temperatures, working with refrigerators or walk-in cold rooms;
  • The handling of hazardous chemicals and materials;
  • Appropriate inactivation, storage and or disposal of infectious blood or tissue materials;
  • Required to be on stand-by for emergency duties and to work beyond normal working hours, whenever the need arises.

Job Summary
Under the supervision of the Senior Human Resource Officer, the incumbent is responsible for assisting with human resource processes such as processing of service records, calculating and updating leave for staff within the hospital and assisting in maintaining personnel files and updating the HR Software.

Qualifications & Experience:
  • Five subjects at GCE O’Level/CXC General Proficiency level inclusive of English Language and Mathematics. plus
  • A minimum of two (2) years’ experience at a clerical level in Personnel Administration or Resource within a public sector organization.

  • Undertaking data entry and scanning of documents and records.
  • Typing confidential documents and files documents relating to personnel matters; makes and/or handles enquiries; assists with investigations.
  • Providing assistance in submitting documentation to the Regional Office in a timely manner such as assumption of duties, resumption from sick, vacation, study and no pay leave, resignation letters, and other key human resources related matters.
  • Providing assistance in preparing and submitting documents to the Supervisor for processing of employees’ retirement benefits in a timely manner.
  • Maintaining a calendar of employee performance evaluations to be conducted, dispatches and collects required forms.

SCOPE OF WORK / BACKGROUND
Jamaica has an estimated prevalence of 1.6% among the general population. Based on modeled estimates and case based surveillance data, it is estimated that 29,000 persons are currently living with HIV in Jamaica. Approximately 84% of the estimated number is aware of their status. This is followed by a large gap in the continuum of care with 43% of those diagnosed on ARVs and 55% of those on treatment being virally suppressed.

The Health Records Officer is to ensure that the basic standards for health records management and patient health information systems are maintained and that optimum patient care within the framework of national priorities and objectives are met.

OBJECTIVE/SUMMARY FOR POSITION

Under the direct supervision of the facility’s Health Records Administrator/Technician, the Health Records Officer has the responsibility to compile process and maintain the health records in a manner consistent with medical, administrative, ethical, legal and regulatory requirements of the health care facility.

MAJOR DUTIES AND RESPONSIBILITIES

  • Protect the security of the health records, ensuring that confidentiality is maintained
  • Gathering of demographic, geographic and health related data and history from all new patients upon registration and three monthly basis, asking all the prompt questions on the form
  • Preparing patient service record for the appropriate clinic
  • Entering of patient’s particular in Daily Register log book to facilitate tally of all registered patients
  • Entering of data on the sentinel forms for each registration
  • Entering of patient record particulars for retrieved records in the record request log book
  • Register all patients for the various clinics
  • Ensure that all health records are assigned to clinic locations
  • Retrieve all health records for appointment
  • Communicate with the patient/client as the need arises in relation to their appointment
  • Ensure good health record keeping is maintained
  • Ensure proper maintenance of all health records by checking all records for
    • Proper identification on all forms
    • Correct sequence of all form by date and time
    • Make a health record for each new patient and retrieve old records for patients who were previously seen at facility
  • Reassemble discharged health records in chronological order
  • Attach loose reports to the record
  • Record all diagnosis or reason for visit in Daily Curative register
  • Make appointments for patients who are for follow-up
  • Answer the telephone
  • Sort and arrange all patient index and records for filing
  • File all health records according to numbering system and indexes in alphabetic order
  • Perform any other health records related duties assign from time to time.

QUALIFICATIONS AND EXPERIENCE
- Good typing skills (at least 30 words per minute)
- Working knowledge in the use of Windows based computers
- Working knowledge in Microsoft Office

- Five (5) subjects in CXC/ GCE including Mathematics and English ( Information
technology / computer studies would be a plus)
