DESCRIPTION
Responsibilities and Duties:
  1. Serves as a primary point of direct administrative contact and liaison with other offices, individuals and external institutions and agencies on a range of specified issues. Organizes and facilitates meetings and other events as required.
  2. Manages and directs telephone and e-mail communications to facilitate the most appropriate responses and actions. Provides accurate and reliable information to enquiries.
  3. Performs research analysis on specific issues as required, and independently prepares non-routine letters and/or reports, which may be highly sensitive and confidential in nature.
  4. Prepares and analyzes Gantt Charts depicting leave schedules for the Department.
  5. Sorts and distributes incoming and outgoing correspondence and prepares or facilitates timely responses as required.
  6. Establishes, maintains and updates files, personnel database and records and facilitates the retrieval of information.
  7. Attends Board Sub-Committee meetings and Senior Management meetings and performs the function of recording secretary. Undertakes research on matters arising out of meetings and new matters to be tabled at subsequent meetings and provides reports as required.
  8. Requisitions supplies, maintenance and other services for the department.
  9. Maintains the calendar for the Head of Section.
  10. Secures confidential documents, directs them to the appropriate persons and that strict confidentiality is maintained in the execution of duties.
  11. Performs other related duties as assigned.

Health Records Clerk (Spanish Town, St. Catherine & St. Thomas - Jamaica) - SERHA

 We invite applications from suitably qualified persons for the position of Health Records Clerk (HTAC/HRT 1) - St. Thomas Health Department & St. Jago Park Health Centre. 


SCOPE OF WORK / BACKGROUND
Jamaica has an estimated prevalence of 1.6% among the general population. Based on modeled estimates and case based surveillance data, it is estimated that 29,000 persons are currently living with HIV in Jamaica. Approximately 84% of the estimated number is aware of their status. This is followed by a large gap in the continuum of care with 43% of those diagnosed on ARVs and 55% of those on treatment being virally suppressed.

The Health Records Officer is to ensure that the basic standards for health records management and patient health information systems are maintained and that optimum patient care within the framework of national priorities and objectives are met.

OBJECTIVE/SUMMARY FOR POSITION

Under the direct supervision of the facility’s Health Records Administrator/Technician, the Health Records Officer has the responsibility to compile process and maintain the health records in a manner consistent with medical, administrative, ethical, legal and regulatory requirements of the health care facility.

MAJOR DUTIES AND RESPONSIBILITIES

  • Protect the security of the health records, ensuring that confidentiality is maintained
  • Gathering of demographic, geographic and health related data and history from all new patients upon registration and three monthly basis, asking all the prompt questions on the form
  • Preparing patient service record for the appropriate clinic
  • Entering of patient’s particular in Daily Register log book to facilitate tally of all registered patients
  • Entering of data on the sentinel forms for each registration
  • Entering of patient record particulars for retrieved records in the record request log book
  • Register all patients for the various clinics
  • Ensure that all health records are assigned to clinic locations
  • Retrieve all health records for appointment
  • Communicate with the patient/client as the need arises in relation to their appointment
  • Ensure good health record keeping is maintained
  • Ensure proper maintenance of all health records by checking all records for
    • Proper identification on all forms
    • Correct sequence of all form by date and time
    • Make a health record for each new patient and retrieve old records for patients who were previously seen at facility
  • Reassemble discharged health records in chronological order
  • Attach loose reports to the record
  • Record all diagnosis or reason for visit in Daily Curative register
  • Make appointments for patients who are for follow-up
  • Answer the telephone
  • Sort and arrange all patient index and records for filing
  • File all health records according to numbering system and indexes in alphabetic order
  • Perform any other health records related duties assign from time to time.

QUALIFICATIONS AND EXPERIENCE
- Good typing skills (at least 30 words per minute)
- Working knowledge in the use of Windows based computers
- Working knowledge in Microsoft Office

- Five (5) subjects in CXC/ GCE including Mathematics and English ( Information
technology / computer studies would be a plus)

REGIONAL DATA ENTRY CLERK

MINISTRY OF HEALTH & WELLNESS
HIV/STI/TB UNIT

JOB PURPOSE

The Data Entry Clerk will be responsible for the input, upload and scanning of data into the
relevant databases to facilitate the monitoring and evaluation functions of the HIV Programme.
The Officer will also support the implementation of the M&E and research activities outlined in
the work plan as well as strengthen the capacity of stakeholders in data entry and data
management by the provision of technical assistance.

KEY OUTPUTS
• Monthly reports on M&E and other activities prepared
• Input/Import and perform validity checks for data completed
• Assist with other statistical updating exercises undertaken by the Unit

KEY RESPONSIBILITY AREAS
1. Perform data entry in several different databases including highly confidential
information
2. Liaison with parish MO(H), Treatment Care and Support Officer and site level Data
Entry Clerk to identify areas for support
3. Provide data entry support to for treatment sites within the region
4. Assist with the gathering/collation of data and data collection forms from various sources
5. Perform validation and verification of data entry forms with documentation of findings
6. Collaborate with professional staff and field officers in the collection, collation, and
analysis of data from M&E system including data for donor reporting.
7. Participate in training workshops to build capacity in Data Quality among site data
entrant
8. Work closely with Regional TCS Officer to prepare reports
9. Perform duplication checks on data entry forms
10. Perform general office administrative duties such as taking messages, preparing reports,
liaising with other component staff
11. Provide progress update on interventions to gap identified
12. Liaison with MOHW/SI Unit on database matters

Job Summary

PMTCT Nurse (HPC/RN 2) - Kingston & St. Andrew, St. Thomas, St. Catherine - SERHA

 The South East Regional Health Authority (SERHA), a statutory body under the Ministry of Health and Wellness, with responsibility for the management and delivery of Public Health Services within the parishes of Kingston & St Andrew, St Catherine, and St Thomas invites applications from suitably qualified individuals to fill the position of PMTCT Nurse (HPC/RN 2).


Job Purpose

The officer will be responsible to ensure that the eMTCT guidelines (Option B+) as stipulated by WHO for HIV and Syphilis positive mothers are being met at the respective Treatment Sites so as to prevent the risk of transmission to the unborn child. He /she will report social, biographic and clinical data relating to HIV, Syphilis and related diseases from perinatal and/or Pediatric and/or Adolescent clients. The PMTCT Nurse is responsible to monitor the elimination of mother to child transmission of HIV and congenital syphilis in the respective parish.

Qualification and Experience

  • Bachelors Degree in Nursing
  • Registered Nurse (RN) with the Nursing Council of Jamaica
  • Certified Nurse mid-wife or Certified Nurse in HIV/AIDS care with at least three (3) years’ experience in general nursing
  • Previous experience in counseling, phlebotomy, research, or public health nursing is an asset.

 Must demonstrate

  • Strict confidentiality at all times
  • The ability to create an enabling environment for all clients accessing care
